68-year-old presenting for colonoscopy
Current medications: metformin, aspirin 325 mg, multivitamin

2 weeks before
Welcome and intake review
Patient receives link from surgery center. Completes intake. Clinician at the surgery center reviews medications and approves personalized plan.
Day -7
1 week before
Stop aspirin 325 mg
Message to the patient: "Per your facility's bleeding-risk protocol, full-dose aspirin is held starting today. Multivitamin and metformin continue as prescribed."
Day -5
5 days before
Avoid certain supplements and OTCs
Message to the patient: "If you are taking these over-the-counter medications or supplements, please hold turmeric, fish oil, vitamin E, and any other bleeding-risk supplements. Hold BC powder and similar combination analgesics. Tylenol is okay."
Day -3
3 days before
Begin low-residue diet
Message to the patient: "Switch to low-fiber foods — white bread, white rice, eggs, lean chicken, broth, gelatin. Avoid nuts, seeds, raw produce, whole grains."
Day -2
2 days before
Last day of solids
Message to the patient: "Eat solid food only until midnight tonight. Continue holding NSAIDs, including BC powder. If you take phentermine, hold starting today."
Day -1
Day before
Clear liquids only + bowel prep
Message to the patient: "All day: clear liquids only — water, broth, apple juice, white grape juice, plain coffee/tea, sports drinks (not red or purple), gelatin (not red). Begin bowel prep solution per your facility's instructions."
Day 0
Procedure Day
Arrive Ready
Message to the patient: "Nothing by mouth today except bowel prep as directed. Hold metformin the morning of (if instructed). Bring your medication list and a ride home (a responsible adult)."
Day +1
Day after
Resume normal medications
Message to the patient: "Restart aspirin 325 mg, multivitamin, and metformin tomorrow morning unless your clinician told you otherwise."

Begin warfarin bridge protocol
Message to the patient: "Stop warfarin today. Begin enoxaparin per your prescribing clinician's bridge instructions. Continue lisinopril and atorvastatin."
Day -1
Day before
Final dose of enoxaparin
Message to the patient: "Last dose of enoxaparin this morning. No food after midnight. Clear liquids okay until your facility's cutoff."
Day 0
Procedure Day
Arrive Ready
Message to the patient: "Hold lisinopril the morning of (per your facility's anesthesia protocol). Take atorvastatin with a sip of water. Bring your medication list and a ride home (a responsible adult)."
Day +1 to +5
Recovery

Message to the patient: "Restart warfarin and Wegovy per the schedule provided by your surgical team."
